ثالثا :بيانات عن موقع الانتهاك واسبابه / Third: Data on the location of the violation and its causes

مكان الانتهاك ( تفصيل اي محافظة اي مدينة – اي حي ) / The place of the violation (detailing which governorate, any city - any district) *

تفاصيل البلاغ ( هل الضحية وصلت اليك لتبلغ ام انت من اقنعها بالبلاغ – ام هي قضية رائي عام) اشرح / Details of the report (Did the victim come to you to report or were you the one who convinced her to report - or is it a public opinion issue) explain

اسباب الانتهاك ( مستخدما نفس الكلمات التي ذكرتها الضحية دون اي تحريف ) *
